Client Records and Documentation

Maintaining accurate clinical records is an important part of providing quality care. Records help document your treatment, support continuity of care, and meet professional and legal requirements.

Your record may include information such as intake paperwork, informed consent documents, assessments, treatment plans, progress notes, scheduling information, billing records, and other documents related to your care.

Records are maintained securely using HIPAA-compliant electronic systems.

Under Texas law, clients generally have the right to request access to their records. There are limited circumstances where access may be restricted if disclosure could reasonably be expected to cause harm or when otherwise permitted by law.

Adult client records are retained for the period required by Texas law following the last date of service. Records for minors are maintained according to applicable state requirements.

If you would like to request a copy of your records or authorize the release of records to another provider, please contact me directly. Written authorization may be required before records can be released.
